Before completing this form, TALK TO YOUR SPONSOR and make sure this is something they’d be interested in doing.

Sponsor must be age 16 or older.
Sponsor must have already received Baptism, First Communion, AND Confirmation.
If married, your sponsor must have received the sacrament of matrimony in the Church.
Sponsor must be a Catholic who leads a life in harmony with the Catholic Church (registered and going to Sunday Mass in a Catholic Church).
You can only pick one person!
Your sponsor cannot be your parent, but he/she can be a sibling, relative, or grandparent.
